Abstract     Introduction. New technical solutions enabled fast development of endoscopy and endoscopic rhinosurgery in the seventies and eighties of the past century. The methods, developed in Europe, were popularised and improved by many authors all over the world and the.field of applications of endoscopic methods has gradually expanded. At the University Department of Otorhiroolaryngology and Cervicofacial Surgery in Ljubljana, since the very start of the routine use of endoscopy in 1987, new endoscopic approaches have been introduced step by step. Our patients. In the period from 1987 to 2002, 4288 patients with nose arcd paranasal sinus pathology were treated endoscopically. In 760 patients, only diagnostic endoscopy was performed, whereas in 3528 patients, we performed 5342 endoscopic operations (in many patients, the endoscopic operation was performed bilaterally). Results. Functional and curative results of endoscopic procedures in our patients are better in comparison to classical methods, including the results concerning rehabilitation, cosmetic consequences and hospitalisation time. In 5342 endoscopic procedures, there were only 38 (0.71%) iatrogenic injuries and complications, all of them without permanent conseguences. Conclusions. At the University Department of Otorhinolaryngology and Cervicofacial Surgery irr Ljubljana we closely follow the development and new possibilities of endoscopic techniques. With routine endoscopy, combined with rationalised use of imaging methods and stepwise introduction of advanced surgical procedures, the diagnosis and the results of surgical treatment of the nose and paranasal sinuses pathology were consistently improved. In our opinion, the endoscopic approach is safe and guarantees the minimum of complications.
Summary     Uvod. Nove tehnične iznajdbe so omogočile skokovit napredek na področju rinološke endoskopske diagnostike in kirurgije v 70. in 80. letih preteklega stoletja. Uporabnost endoskopske metode, ki so jo razvili v Evropi ter izpopolnili in popularizirali številni avtorji po vsem svetu, se neprestano širi. Na Kliniki za otorinolaringologijo in cervikofacialno kirurgijo v Ljubljani smo pričeli z rutinsko endoskopijo leta 1987 ter postopoma širili indikacije in uvajali nove endoskopske operacije. Naši bolniki. V obdobju od leta 1987 do 2002 smo endoskopsko obravnavali 4288 bolnikov z boleznimi v področju nosu in obnosnih votlin. Pri 760 bolnikih smo naredili le diagnostično endoskopijo, 3528 bolnikov smo operirali in pri njih naredili 5342 endoskopskih operacij (pri številnih bolnikih je bila endoskopska operacija narejena obojestransko). Rezultati. Pri vseh endoskopskih posegih so rezultati boljši kot pri klasičnih operacijah, tako v funkcionalnem in kurativnem smislu kot tudi glede rehabilitacije, kozmetskih posledic in časa hospitalizacije. Pri 5342 operacijah je bilo 38 (0,71%) jatrogenih poškodb in zapletov, vendar brez trajnih posledic. Zaključki. Na Kliniki za otorinolaringologijo in cervikofacialno kirurgijo v Ljubljani sledimo razvoju novih možnosti in tehnik v endoskopski rinokirurgiji. Z rutinsko endoskopijo, postopnim uvajanjem novih endoskopskih operacij in smiselno uporabo slikovnih metod smo izboljšali diagnostiko in rezultate kirurškega zdravljenja patoloških procesov v področju nosu in obnosnih votlin. Tak pristop po našem mnenju tudi jamči najmanjše možno število operativnih zapletov.
