Abstract     Background. Although complications in cholecystectomy are infrequent, iatrogenic lesion of the extrahepatic bile ducts is one of the most serious complications in surgery, owing o the very difficult nature of correcting it. Methods. The authors report on iatrogenic lesions of bile ducts occured in Teaching hospital Maribor between l980 - l989. In 10 years, 6646 cholecystectomies were done (1927 men, 4179 women, average 52.6 years, range 14-91). Results. In 8 cases (six women, two men, average 62 years, range 28-75) iatrogenic lesion of the bile duct occured. Seven lesions were detected and treated in the case of surgery, once the lesion was overlooked. All lesions occured before intraoperative cholangiography was carried out. In partial lesion direct suture was applied once and three times suture over the T tube. In complete transsection termino-terminal reconstruction over the T rube was performed. ln patient with overlooked lesion after six months stricture developed. She was reoperated and biliodigestive anastomosis by Roux was performed. ln two female patients reoperations were reguired after primary reconstruction over T tube. In both cases finaly biliodigestive anastomosis after Roux was done. All patients are now without complaints. Conclusions. Authors concluded that iatrogenic bile duct lesion is a serious complication that should be reconstructed by experienced surgeon only.
