Abstract     Capabilities of available rehabilitation facilities for stroke survivors are modest and unequal in Slovenia. Although stroke rehabilitation should begin as soon as diagnosis of stroke is established and lifethreatening problems are under control, rehabilitation during acute hospitalization is organized only in some hospitals, despite the fact that 80% of patients are admitted to hospitals after cerebrovascular accident. Stroke rehabilitation can be conducted in inpatient rehabilitation hospitals, in nursing facilities with rehabilitation programs, in outpatient facilities, or in the home. In Slovenia about 40% of all stroke survivors are included in some kind of rehabilitation. Usually the rehabilitation programs are too short and insufficient. The greatest problem is transition to the community, especially for the most severely disabled, because of the lack of the community resources and supports.
