Abstract     Transport of patients or injured in the air has its roots in the military organization of casualty. Dr. Larrey performed the first transport of casualties in a specially modified carriage in 1792 and the first balloon transport was performed as early as 1870 during the Franco-Prussian war. Today we view air transport of casualties as an integral part of the organized emergency medical care services. Despite their long tradition, however, the helicopters have yet to receive the proper recognition in the field of emergency medicine, at least as far as an organized scientific approach and proper analysis are concerned. This article places under scrutiny the dilemmas facing the organizers of the emergency medical care services and go hand-in-hand with both financial and professional limitations.
