Abstract     In the recent decade, the survival of the patients with advanced colorectal cancer has improved considerably, which is mainly due to the application of new cytostatics. At present, the mean survival of these patients is longer than two years because of the use of combinations of different drugs and of different regimes of chemotherapy. However, the majority of these patients still receive the standard palliative chemotherapy. The objectives of our study were to test the effect of neoadjuvant chemotherapy with irinotecan, 5-fluoruracil (5-FU) and calcium folinate and to establish the percentage of resectability in the patients with primary unresectable liver metastases from colorectal cancer. We treated 25 patients by the combination with irinotecan, 5-FU and calcium folinate. This combination was applied as primary treatment in b/25 patients and as secondary treatment in 19/25 patients. Chemotherapy was successful in 14/ 25 patients and 6/25 patients underwent radical surgery. After neoadjuvant chemotherapy with irinotecan, 5-FU and calcium folinate, a considerable number of patients with primary unresectable colorectal cancer can undergo radical surgical treatment. The effect of such treatment on the survival of these patients could not have been established so far because the observation period was too short.
