Abstract     On plain abdominal image free air may be seen in three positions: mostly in standing position, supine or on the left decubitus in patients, who are not being able to stand. There are many causes for free abdominal air e.g. illness, injuries and therapeutic or diagnostic procedures. On erect plain abdominal image, free air is seen as a dark crescent under hemidiaphragma and on left decubitus as a crescent above the liver. On supine image free air is seen above or under the liver, as a double wall sign or may be visualized in the falciform ligament.
