Abstrakt		The majority of thoracic spine injuries are treated with conservative measures. Operative therapy is indicated in fractures with neurological deficit, mechanical instability, major deformity or pain. Posterior approach is ususally used. The fracture is reduced with the help of ligamentotaxis and fixed with internal tixator. Anterior approac enables direct decompression of the spinal canal and reconstruction of the anterior and middle spine coloumn with tricorfical bone graft from the iliac crest or with metallic cage.
