Abstract     From 1991 to 2001 147 patients with thoracolumbar spine fracture were treated at the Department of traumatology in General and teaching hospital Celje. The posterior approach was the preferred route and in every case internal fixator was used for stabilisation. There were 26 (18%) polytraumatized patients. According to Magerl classification 78 patients were classified as type A, 51 as type B and 18 as type C. At admission neurological status of 131 (89%) patients was classified as ASIA E and at discharge from hospital there were 138 (94%) neurologically intact patients. Deterioration of neurological status occurred in 9 (6%) patients. We had one deep infection, in two patients pedicular screws were reinserted, 13 patients had hardware failure. 100 patients were followed up and re-evaluated at least one year after injury 80 patients had satisfactory relief of pain, and 88 were satisfied or very satisfied with their results. 52 were back to work. The results of the treatment in our group are comparable to the published. They could be improved by using ventral approach in selected cases. Gradually we are adopting the trend of minimal invasive surgery.
