Abstract     The measurement of intima-media thickness of carotid arteries using high-resolution B-mode ultrasonography is an important index for determining early atherosclerosis. An increased thickness of carotid intima-media has been shown to be associated with an increased risk for myocardial infarction and stroke. Clinical practice has shown great interest in detecting preclinical changes of atherosclerosis which could predict future cardiovascular events better than traditional risk factors and could better identify those individuals who would benefit the most from aggressive pharmacologic treatment. We have tried to make a draft protocol for ultrasonographic evaluation of intima-media thickness in clinical practice, but have realized that there are still a lot of unanswered questions about the standardization of measurements, as well as in defining the normal and the pathological thickness of intima-media. Therefore, before actually using the method in clinical decision-making it will be necessary to resolve the above-mentioned questions with further clinical research.
