Abstract		Transplantation of organs and tissues is in spite of huge advance in surgical techniques still ambiguous. The major disadvantage of transplantation threatens from immune response oriented toward the foreign tissue antigens. In the paper problems linked to the heterogeneity of major histocompatibility antigens and type of immune response directed against them are presented. In continuation of discussion results of two recent studies on immune response in patients succeeding the kidney transplantation are displayed. In srrmmary short overview of efficacy of transplantation is given together with review of medical, psychosocial, and economic problems associated with organization of transplantation service.
