Abstract		Though aggressive medical therapy has long been the therapeutic mainstay for patients with advanced heart failure, the use of left ventricular assist devices (LVADs) is a potentially safe and effective alternative. LVAD therapy is effective in ameliorating symptoms of end-stage heart failure and is thus a favorable alternative for symptom relief. Furthermore, in light of the available evidence, it appears that LVAD therapy is associated with improved outcome of patients with end stage heart failure. LVADs have also proven useful in preventing or reversing heart failure by inducing neuroendocrine modulation and reverse remodeling. Finally, LVAD therapy may also allow for better optimization and more efficient implementation of medical therapy regimens in patients with advanced heart failure.
