Abstract     Direct coronary (DS) stent implantation without prior balloon dilatation is a new treatment strategy for coronary artery stenoses. DS was enabled by the technical development of stent designs and delivery systems with lower crossing profiles. DS compared to conventional stenting (with ballon predilatation) has few advantages as well as disadvantages. The purpose of the study is the evaluation of the stent size for DS. Twenty-two patients with angina pectoris (stable, unstable) and significant right coronary artery (RCA) stenosis were included in the study (15 male, 7 women 59±9.3 year). DS of RCA is feasible, safe and faster if all pitfalls of DS are taken into account. The possibility of underestimation of the real RCA size and the selection of appropriate stent size appear to be the most problematic issues in author's view. Coronary angiography with nitroglycerin and operator's experience are very helpful in the decision making. The decision of DS has to be individual. In a doubtful cases conventional stenting is preferred.
