Abstract     Introduction: Immediate transfer of the patients with an acute myocardial infarction from the ground to the primary PTCA in prehospital setting of medical emergency unit Celje has run routinely for the past two years. If acute coronary syndrome is accompanied by cardiac arrest and subsequently a need for cardiopulmonary resuscitation, a decision whether the patient shoud be immediatelly transfered to primary PTCA or not is difficult. Case report: 58-years old man came on medical examination to a prehospital emergency outpatient unit because he had a apin in his left arm. ECG has been done and it had shown minimal ST-segment elevation in the leads of inferior wall. He collapsed and become unconscious, pale and cyanosed during medical examination. He was without vital signs. rhythm on the monitor was ventricular fibrillation. He was four times defirillated during cardiopulmonary resuscitation, artificialy ventilated by a mask and a self-inflating bag, an external chest massage was performed. After the fourth defibrillation sinus rhythm had been established, the pulse had been palpable, he started to breathe spontaneously. He become conscious, he was able to communicate. ECG has been done again, but the patient again lost his consciousness and again on the monitor was ventricular fibrillation. He was further resuscitated by the external massage, intubation, defibrillation. After the tenth defibrillation sinus rhythm had been established, the pulse had been palpable, and it had been possible to measure blood pressure. (Abstract truncated at 2000 characters)
