Abstract     Primary Sjogren's syndrome is the second most common, chronic systemic autoimmune disease, characterized by progressive inflammatory destruction of exocrine glands. In addition to accepted tests (unstimulated salivary flow test, salivary gland scintigraphy and sialography), other methods for confirmation of salivary gland affection are being studied. Among them ultrasonography of major salivary glands seems attractive as a non-invasive, inexpensive, non-irradiating method. Our study group designed a semi-quantitative scoring system. The diagnostic value of the novel scoring system was verified in a cross-sectional study on more than two hundred patients suspected of having pSS and the reproducibility was assessed in a study with two in diagnosis blinded observers. Results showed its high specificity and good reproducibility.
