Abstract     Besides impaired skin barrier function, atopic diathesis and irritation of cosmetic agents, microbial changes are probably also important in the pathogenesis of perioral dermatitis. Among them, Demodex folliculorum has been suspected to play a pathogenic role. However, its role in this disease has received comparatively little attention in the studies. In the presented study, we investigated the prevalence and density of Demodex folliculorum with the use of standardized skin surface biopsy in 82 female patients with perioral dermatitis and 70 control female subjects. We have found significantly higher mite prevalence and density in patients who had received previous topical steroid therapy compared with patients who had not received topical steroids. In the latter group mite prevalence and density did not differ significantly from that of the control group. Mite density increased significantly with the lenght of treatment with topical steroids. Our results suggest that increased density of Demodex folliculorum in perioral dermatitis is a secondary phenomenon. In our opinion, it has no major role in the pathogenesis of this disease but it may contribute to aggravation of inflammarion in patients treated with topical steroids.
