Abstrakt     The incidence of skin melanoma in Slovenia is rising. Involvement of regional lymh nodes is one of the most important prognostic factor by such patients. Elective lymphadenectomy does not represent any benefit for patients with skin melanoma. Sentinel node biopsy helps to establish the involvement of regional lymph nodes at the time of initial diagnosis. Skin melanoma metastases to regional lymph nodes by defined lymphatic pathway from the primary tumor to the first or sentinel node. Studies showed that disease progresses gradually and in case of negative sentinel node there were no metastases in regional lymph basin detected. Biopsy of sentinel node helps to identify 20% of patients who have micrometastase in regional lymph nodes at the time of initial diagnosis. Those patients have benefit from regional lymphadenectomy. 80% of patients by whom the sentinel node is negative therefore are spared the regional lymphadenectomy and subsequent morbidity. Sentinel node biopsy by patients with skin melanoma is already a standard procedure of staging.
