Abstract     Chronic diseases are long lasting conditions that cannot be improved and are rarely cured completely. They have a profound effect on the physical, emotional and mental well-being of individuals, often making it difficult to carry on with daily routines and relationships. However, in many cases, deterioration in health can be minimized by good care. Chronic disease management is at the fore of health care issues and health care planning primarily because of the complexity and co.morbidity of chronic disease. It is important to know the outcome of treatment and where improvements can be made. It is very time consuming to teach patients enough to make them co-managers. The biggest barrier is noncompliance . There is a danger of fragmentation of care because many patients have more than one chronic problem.
