Abstract     Atopic dermatitis is very common skin disorder in well developed countries. Pruritic skin lesions appear on different sites of predilection in different age groups. The appearance of skin leisons are changed by superinfectons cased by bacteria or viruses. Topical steroids have been the conventional therapy for atopic dermatitis for 50 years. They act on broad spectrum of immune cells but also interfere with other skin cells. Tacrolimus ointment and pimecrolimus cream are new topically effective macrolides. Both were found safe and effective for the treatment of atopic dermatitis in children and adults. They inhibit inflammatory cytokines transcription and other inflammatory cells through inhibition of calcineurin. Local risk of corticosteroids do not occur, nor do the systemic side effects. When used in the first signs and symptoms of the disease they have significant advantages over conventional therapy and steroid sparing effect.
