Abstract     Plain abdominal film is still the most widely used and sometimes the only diagnostic method in emergency departments in patients with symptoms of acute abdominal pain. Opinions differ whether it should be used routinely in all patients or only in cases of suspected gastrointestinal obstruction, perforation or renal colic. Controversies exist also in which position abdominal plain film should be taken and which radiologic signs are diagnostic. Because of supposedly very low sensitivity and specifity some authors suggest, that plain abdominal film should be supplanted by computerised tomography. In this case also the opinions differ. Before the patient reaches the radiologic department, the diagnostic possibilities should have been reduced to just a few likely entities and in many cases diagnostic imaging is not necessary. For viscus perforation or obstuction the merits of the plain abdominal film are still widely recognized. Angiography in cases of intestinal bleeding, with possibilty of embolization, and computerised tomographic angiography are used when there is strong clinical suspition of vascular disease.
