Abstract | Iron is present in almost all living beings. For humans it is an essential trace element, which is, however, potentially toxic to cells in large quantities. Iron homeostasis is regulated by precise complex mechanisms, including the entry of iron into enterocytes, formation of stores in the form of ferritin and transportation of iron to other cells. Iron is present in many biochemical reactions of the immune system, central nervous system and muscles, in energy conversion in mitochondria and in binding and transportation of oxygen to all parts of the body. Malnutrition leads to iron deficiency, anaemia and impaired mental development in children. Iron overload, which is genetically conditioned in most cases, leads to parenchymal organ damage. During their daily work, paediatricians are more often confronted with iron deficiency, but they also need to be aware of iron overload. Both disorders demand timely recognition and intervention. | |
Summary | Železo se nahaja v vseh živih bitjih. Za človeka je esencialni element v sledovih, v velikih količinah pa potencialni strup za celice. Homeostazo železa vzdržujejo natančni kompleksni mehanizmi, ki zajemajo vstop železa v enterocite, tvorbo zalog v obliki feritina in prenos eleza v druge celice. Železo sodeluje v številnih biokemijskih reakcijah imunskega sistema, osrednjega živčevja, mišic in pri tvorbi energije v mitohondrijih, pri vezavi in prenosu kisika v vse dele telesa. Pomanjkljiva prehrana vodi v pomanjkanje železa, slabokrvnost in motnjo umskega razvoja pri otrocih. Preobremenitev z železom, ki je najpogosteje dedno pogojena, vodi v okvaro parenhimskih organov. Pediater se pri svojem delu najpogosteje sreča s pomanjkanjem železa, vendar zahteva njegovo pozornost tudi preobremenitev z žlezom. Obe motnji je treba pravočasno prepoznati in ukrepati. | |
