Abstrakt     BNP (brain natriuretic peptide, B-type natriuretic peptide) has diuretic, natriuretic and vasodilating effects. The precursor of a biologically active form is produced in cardiac myocytes of both ventricles. NT-proBNP and active molecule BNP are released into blood. In healthv subjects their concentration in blood is low; however, it inereases markedly in congestive heart failure as a result of increased myocardial stretch. Serum concentrations of BNP and NT-proBNY are correlated to the degree of myocardial damage and to NYHA class. BNP and NT-proBNP have very good negative predictive value for ruling out acute heart failure as a cause of acute dyspnea. Increased serum concentrations of BNP and NT-proBNP contribute to diagnosing congestive heart failure and left ventricle dysfunction. They also carry prognostic information in congestive heart failure. After successful treatment of congestive heart failure serum BNP and NT-proBNP decrease. Currentlv, BNP and NT-proBNP are not yet routinely used in everyday clinical practice for treatment monitoring in patients with congestive heart failure.
