Abstract     Effective staging and treatment of cervical metastases are the most imporWnt factors influencing the survival of patients with head and neck carcinomas. Analyises through evolution of neck dissections showed that in majority of cases the recurrences are within operation field so it could be safe enough to use functional (selective and modified) techniques not only in staging but also in curative intent in limited nodal disease. There is increasing evidence that in nodal extracapsular spread postoperative concomitant radiochemotherapy enables better regional control and also improves disease free survival.
