Abstract     In the introduction the authors describe the basic anatomy of head and neck sympathetic system that is important for understanding of Horner's and both Raeder's syndromes. The definitions for all entities are given. One case report with unusual presentation of acute sinusitis is presented. The course of disease was complicated by Raeder's syndrome II. The treatment and outcome are also described. The discussion offers further diagnostic possibilities for distinguishing these syndromes and tries to explain the pathophysiology of sympathetic involvement in acute sinusitis.
