Abstract     Patients who suffer an osteoporotic vertebral or hip fracture should be diagnosed with severe osteoporosis regardless of the DXA result. Furthermore, sečondary causes of osteoporosis and other bone diseases, which are more common than usual in this case, should be excluded before treatment. Then the doctor can choose from among all antiresorptive antiosteoporotic medications and strontium ranelate, combined with vitamin D and calcium supplementation. The choice should also be influenced by the specific characteristics and preferences of the patient, and by fracture type. If the same patient suffers another osteoporotic vertebral or hip fracture after at least one year of appropriate treatment, he/she can be offered a trial with teriparatide.
