Abstrakt     The optimal management of early gastric cancer should be stage-oriented, with the lymph node status as the main criterion. Nodal status can be assessed preoperatively using modern radiological imaging techniques, sentinel node biopsy, molecular or genetic markers of the tumour, and computer analysis. The aim of this study was to evaluate the feasibility and accuracy of preoperative prediction of lymph node status based on computer analysis. We used the WinEstimate computer programme, and cons-tructed our own computer model using data of patients with gastric cancer treated in the University Medical Centre Ljubljana between 1993 and 2003. The prediction of two computer models was then tested on 110 patients treated between 2004 and 2005 in the same manner as the patients from the database. The goal of the study was to compare the lymph node status in the control group predicted by the computer programme with the real data collected postoperatively. The accuracy rate of computerized preoperative predictions of N0/N1 status with the first computer model (WinEstimate) was 91% (sensitivity 94% and specificity 87%), and with the second computer model, 86% (sensitivity 91% and specificity 79%). Accuracy rates of preoperative predictions of nodal status are very high and, with some restrictions, can be used to select stage-appropriate therapy in patients with early gastric cancer. The applicability is improved by employing this new approach with standard diagnostic methods.
