Abstract     Background. Idiopathic autoimmune haemolytic anaemia due to warm reacting autoantibodies (AIHA) is a rare disorder. The majority of patients are overage 40. In a retrospective study the effect of corticosteroids, cyclophosphamide and splenectomy was evaluated for 22 patients admitted to the Department of Haematology. Methods and results. The mean age was 61 years and the sex ratio male/female was 6/16. The mean haemoglobin level was 74g/L, the mean reticulocyte number 459 x 10 9/L. All patients had positive direct antiglobulin test. 21 patients were treated by corticosteroids O.5-1 mgper kg BW. Four patients received cyclophosphamide. Splenectomy was done in one patient. After splenectomy, the direct antiglobulin test was negative. The treatment with corticosteroids was successful in 20 patients. Nine patients relapsed. Side effects of corticosteroids were diabetes, oedemata and psychosis. Conclusions. The initial treatment of AIHA is well established. Standard first-line therapy involves corticosteroid. Second-line therapy is usually splenectomy or the use of other immunosuppressive drugs (cyclophosphamide).
