Abstract     Background: Treatment of aggressive periodontitis represents a challenge for clinicians as there are no established protocols for efficient control of this disease. Materials and methods: Ten subjects with untreated aggressive periodontitis were randomly divided into two groups. Scaling and root planing (SRP) was performed in both groups. The test group received additional photodynamic therapy. The control group received antibiotics in addition to SRP. Clinical parameters were assessed at baseline and 3 and 6 months after the end of treatment. Results: Mean probing depth decreased from 3.85 ± 1.88 mm at baseline to 2.85 ± 1.08 mm at 3 months and 2.95 ± 0.04 mm at 6 months in the test group, and from 3.10 ± 0.04 mm to 2.48 ± 0.86 mm and 2.47 ± 0.04 mm respectively in the control group. The corresponding decrease in clinical attachement loss was from 4.05 ± 0.05 mm to 3.20 ± 1.40 mm and 3.20 ± 0.05 mm in the test group, and from 3.40 ± 0.05 mm to 2.92 ± 1.26 mm and 3.00 ± 0.05 mm in the control group. The percentage of sites with bleeding on probing decreased from 47.6% to 10.8% and 4.0% in the test group, and from 13.7% to 7.0% and 5.3% in the control group. The observed changes in all three parameters were statistically significant (p < 0.05). Conclusion: Both photodynamic therapy and antibiotics, used as an adjunct to SRP in the management of aggressive periodontitis, result in improvement of clinical parameters 3 and 6 months after the end of treatment.
