Abstrakt     Background. Type 2 diabetes mellitus (DM) is a major risk factor for the development of coronary artery disease (CAD). Myocardial infarction (MI) is four times more frequent in diabetic than in non-diabetic patients and is associated with increased inflammation, higher rates of complications and higher mortality. We investigated cardiovascular risk factors in diabetic patients with a recent MI and diabetic patients without clinical CAD. Furthermore, we examined the association between recent MI and high sensitivity C-reactive protein serum levels. Patients and methods. 495 Patients with DM were included in the study, among them 169 patients with a recent MI and 326 patients without clinical CAD. Patients with MI were included in the study 1 to 9 months after the awte event. Results. Among patients with MI there were more males (67% vs. 46%, p < 0.001), they were more likely to smoke (41% vs. 12%, p < 0.001) and had higher serum levels of high sensitivity CRP (7.4 +- 16.1 mg/I vs. 3.9 +- 4.5 mg/l; p = 0.01), total cholesterol (5.7 +- 1.6 mmol/I vs. 5.3 +- 1.3 mmol/l; p = 0.01) and LDL cholesterol (3.6 +- 1.5 mmol/l vs. 3.2 +- 1.0 mmol/I; p = 0.001). We didn't find any significant differences in the prevalence of arterial hypertension, blood pressure values, body mass index, waist circumference, and serum levels of HbA1c, triglycerides and HDL-cholesterol Conclusions. Diabetic patients with recent MI have less well controlled cardiovascular risk factors and more evident inflammation than diabetic patients without clinical CAD.
