Abstract     The mortality in chest trauma in multiple injured patients especially with additional head injuries is still high (40%). Autor emphasised the importance of rapid diagnosis based on clinical examinations and therapeutic interventions in form of tube thoracocentesis and pericardiocentesis. Cardiac trauma sometimes requiring immidiate surgical intervention. A rational approach by sistematisation of criteria for emergency thoracotomy is presented. The signs and simptoms, pathogenesis, diagnosis and therapeutical approach is recomended for lung contusions, lacerations and tracheobronchial rupture, as well as open and blunt heart injuries and aortic arch disruption. Early diagnosis and treatment of the heart injuries is associated with a low morbidity and mortality. Separately are discussed chest wall and diaphragm injuries.
