Abstract     Basic information about the patient, given both in a form of a spoken account and a written report, is essential to the continuity of the process of patient care, and should always be obtained on admitting the patient to an emergency clinic. The required details about the accident and the sustained injuries may be given either by the accompanying persons or the patient's relatives, by the ambulance staff, or, if his health status permits him, by the patient himself. The author defines those history data that are indispensable to prevent possible errors of diagnosis and therapy, and ensure proper patient management. The provision and exchange of clinical details and reasons for admission of and emergency case are strictly the domain of the health care personnel, i.e. a physician, nurse, medical technician, and ambulance staff. In addition to obtaining a spoken account of the accident, a standard form should be competed, which then follows the patient through all phases of emergency medical care, to his final destination in the hospital.
