Abstract     Background. A case of traumatic pneumocephalus was observed in a patient with head injury accompanied by a superficial lesion that was at first sight of no importance. Methods. Initially the patient did not require the medical assistance as he bad not been aware of his head injury. At the standard skull X-ray examination there were only slight signs of pneumocephalus with no bone lesions. The brain CT exam with the standard technique confirmed the diagnosis of pneumocephalus, but only the CT with the technique of high resolution (HRCT) revealed the bone lesions. Conclusions. If the head injury with a penetrating object had presented no signs of pneumocephalus, the CT examination technique would have been of foremost importance.
