Abstract     Long term results (five year patency) of periphral arterial bypass using PTFE grafts have, by classic technique of anastomosis (graft on artery), remained poor in comparison with those using saphenous vein grafts. Anastomoses with vein patches have enabled considerable improvement. We are presenting a simple modification to the Taylor operative technique of vein patching, involving the most possible minimal contact of the PTFE graft with the artery. After some years of using several different techniques, in the last five years (1990-1994) at the our department our new modified technique was applied only. In the defined period 63 (average 61 years, range 39-73, 39 male, 24 female) patients with CCI underwent limb salving operation with our modified technique. Overall patency rate was 61%. 19 patients died during the follow up ad seven were lost. In conclusion we could state that vein patching of the PTFE graft anastomosis brings overal patency rates close to those observed in patients where vein alone was used. We think that our technique is simple and has some advantages, the results should be confirmed on larger series.
