Abstract		In the present abstract the main characteristics regarding pathogenesis, clinical significance and the possibilities of ventricular tachyarrithmia treatment in the setting of coronary artery disease are presented. Clear differences between those related to acute myocardial ischemia in the setting of acute conorary events and those related in the setting of chronic postinfarctional coronary diseases are delineated regarding their pathoanatomic substrate. In the case of acute coronary events, a broad spectrum of pathogenic mechanisms of ventricular tachycardias related to acute myocardial ischemia is described. In the second case, the pathoanatomic substrate related to myocardial fibrotic changes is described as the base for reentrant focus of monomorphic ventricular tachycardias in the chronic phase of coronary artery disease 48 hours after the acute conorary incident. The abstract stresses the importance of clinical electrophysiologic interpretation and understanding of pathogenesis as the modern therapeutic and diagnostic possibilities in the setting of coronary artery disease.
