Abstract     Large healt institutions which are composed of several smaller professionally or different organizational units can be managed rather centrally with great mandates of managing director (e,g, Medical centre, Ljubljana) or as more less independent (financially) organizational units (e.g. former Basic organizations of associated labour (B.O.A.L.). Both organizational forms have good as well as their bad aspects. This project tried to evaluate good and bad aspects of both organizational forms on the example of Medical Centre Ljubljana. It has been established that in spite of some obvious deficiencies of management decentralization there are still more wellfounded reasons for a financial economic independence of organizational units that neverthless remain a constituent part of the institution.
