Abstract     Analgesia and sedation are necessary during the critical care transport. Need for the pain relief and anxiolysis is obvious in conscious patient who is in distress. Beside the patient discomfort, there are other harmful influences, which make analgesia and sedation unavoidable. On the other hand, with the use of benzodiazepines, intravenous anesthetics and opioid analgesic we expose the patient to a numerous complications, respiratory depression and haemodynamic disturbances are the most important ones, we should be aware of. Titration of dose and careful monitoring is necessary, if we want to provide good patient comfort and avoid this complications. Different approach is used in primary and in a secondary transport, because we have to deal with different kind of patient. Provider should be aware of this differences and should select suitable analgesic and sedative drug according to the patients' needs and his condition. When using benzodiazepines, iv. anesthetics and opioid analgesics, we should be always prepared to start with the ALS if complications arise.
