Abstract     Chronic gastritis is a common disease. It is most frequently associated with Helicobacter pylori (HP) infection. Nonatrophic chronic gastritis may evolve to atrophic chronic gastritis, where intestinal metaplasia (IM) is frequently observed with occasional development of dysplastic epithelial changes and adenocarcinoma, as final event. There are different oppiions about the mechanism by which IM develops and about the designation of IM as premalignant state. Researches of IM are performed also on molecular level. The contribution reviews the results of that studies: they show that mutations readily occur in hyperplastic polyps and IM. The results of studies are not unique. Beside, infection caused by HP is possibly mutagenic itself. Hyperplastic lesions can include hyperplasia of foveolar layer, glandular layer and muscularis mucosae, or can be a combination of these. They occur in combination with reactive and HP-associated chronic gastritis, in the vicinity of chronic erosions, and in peptic ulcer disease. Some hyperplastic lesions, especially hyperplastic mucosa, may be found in the vicinity of gastric cancer. Endogenous and exogenous factors that cause hyperplastic lesions are still not well established. Cytokines and growth factors released during inflammation and healing probably have an important role, as well as HP infection.
