Abstract     This review concentrates on practical management with the main classes of the antiparkinsonian drugs from the perspective of the disease progression. Parkinson's disease cannot be prevented; however, some studies conclude that deprenyl may slow down the progression of the degenerative process. Symptomatic treatment may be initiated with deprenyl or a dopamine agonist in younger, and with a conventional and/or slow-release preparation of levodopa in older patients. As the disease progresses, the timing, dose, and formulation of levodopa must be individualized and new dopamine agonists should be introduced, apomorphine being particularly suitable for rapid alleviation of disabling "offs".
Summary     Pregledni članek osvetluje praktične vidike uporabe najbolj razširjenih skupin zdravil v različnih fazah Parkinsonove bolezni. Bolezni ne moremo preprečiti, nekatere študije pa nakazujejo, da bi deprenil utegnil upočasniti napredovanje degenerativnega procesa. Proti simptomom lahko pričnemo zdravili pri mlajših bolnikih z deprenilom ali dopaminskim agonistom, pri stareših pa s konvencionalnim ali novejšim "slow-release" preparatom levodope. Z napredovanjem bolezni bomo morali za posameznega bolnika prilagojevati čas jemanja zdravil, odmerke in obliko levodope. Običajno bo treba dodati dopaminske agoniste, od katerih je zlasti apomorfin primeren za hiter prehod iz mučnega "izklopljenega" stanja.
