Abstract     In clinical work we are often challenged by a problem of insufficient efficacy of solute removal by hemodialysis. Blood flow rate influence on hemodialysis solute removal is incompletely explained. There are also no available experimental data on dialysate flow rate influence on hemodialysis solute removal with modern high-flux hemodialyzers. The objectives of our prospective clinical study were as follows:- to try to achieve urea reduction ratio of 65 % during 4-hour bicarbonate hemodialysis, using hemodialyzer F 60, dialysate flow rate of 300 ml/min, and increasing blood flow rate from 200 ml/min to 300 and 400 ml/min. - to evaluate during the same hemodialysis procedure which influence on hemodialysis removal of solutes with small molecules is stronger: (a) influence of increasing blood flow rate at constant dialysate flow rates or (b) influence of increasing dialysate flow rate at constant blood flow rates. - to conclude whether during the same hemodialysis procedure increasing blood flow rate or increasing dialysate flow rate also increases hemodialysis removal of solutes with large molecules (beta-2-microglobulin). We performed 9 different 4-hour bicarbonate hemodialysis procedures with each of 8 included hemodialysis patients. These 9 hemodialysis procedures differed only in dialysate (300, 500 and 800 ml/min) and blood (200, 300, 400 ml/min) flow rates. We used hemodialyzer F 60, a high-flux polysulfone hollow-fibre hemodialyzer. (Abstract truncated at 2000 characters.)
