Abstrakt | For decades, high-frequency electrical current has been used to incise or coagulate tissues to obtain hemostasis. Recently, the technique has been largely employed in endoscopic surgery. Polarity (unipolar and bipolar) refers to the number of electrical poles used in electrocautery. The use of high-frequency electrical current, however, may produce thermal injury away from the plane of cutting, or as a result of contact with other metal instruments, detachment of the ground plate, or emission of sparks. Burns of the skin or heart rhythm disturbances may occur. A bipolar cautery has been increasingly used to reduce or even eliminate these potential risks. In some countries, only the use of bipolar electrocautery is recommended for endoscopic surgery. | |
Izvleček | Visokofrekvenčni tok se že desetletja uporablja v kirurgiji za rezanje tkiv in ustavljanje krvavitev, veliko pogosteje pa se uporablja v endoskopski kirurgiji. "Polarnost" (monopolarna in bipolarna) označuje število električnih polov na mestu uporabe električne energije. Uporaba toka ima lahko neželene termične poškodbe zaradi: delovanja elektrike zunaj področja preglednosti, neposrednega stika z drugim kovinskim inštrumentom, napajanja, iskrenja, opeklin, vpliva na srčni utrip. Zaradi teh in drugih možnih posledic se vse pogosteje uporabljajo bipolarni inštrumenti, s katerimi se število zapletov zmanjša ali se jim celo izognemo. V nekaterih državah priporočajo izključno uporabo biopolarnega toka za endoskopske posege. | |
