Abstrakt     Single fibre EMG (SFEMG) allows for highly accurate determination of time when the single fibre action potential (SFAP) passes the electrode. Time measurements with high resolution made possible studies of ephaptic transmission within the muscle. This has permitted, for example, elucidation of the nature of complex repetitive discharges. - SFEMG is the most sensitive technique for diagnosing disorders of neuromuscular junction (NMJ). THe jitter reflects the safety margin of a NMJ. Changes in jitter with activity may provide a clue for differentiating between the pre- and postsynaptic nature of abnormality. - The jitter of late responses can be used to differentiate between recurrent and reflex responses and also between monosnaptic, oligosynaptic and polysynaptic responses. Jitter of motor evoked responses identifies the direct and oligosynaptic corticospinal connexion to the lower motor neurone. Fibre density (FD) is a sensitive parameter for detecting changes in architecture of the motor unit.
