Abstrakt     We investigated nociceptive reflex mechanisms in the upper limb of live healthy volunteers using transcranial magnetic stimulation (TMS) conditioned by high intensity electrical stimulation to the tip of the index finger at various interstimulus intervals (ISI). Motor evoked potential (MEP) amplitudes were profoundly suppressed in thenar eminence muscles at all ISI with a maximum at ISI 60 ms. They were less inhibited in the abductor digiti minimi muscle at ISI below 80 ms and augmented at longer ISI. MEPs were facilitated in bicepsbrachii and less in tricesbrachii at ISI 100 to 160 ms with a maximum at ISI ms. MEP latencies, however, were shortened at all ISI above 40 ms in all four muscles, with maxiumum facilitation at ISI 60 to 80 ms. Nociception produces a differential modulatory effect on MEP amplitudes in different motoneurone pools in the human upper limb. A distinct mechanism, however, is evident for MEP latency modulation.
