Abstrakt     These has been a gradual evolution in the surgical technique for organ procurement from the meticulous in-vivo dissection and removal of warm organs to the modern non-touch, enbloc removal of multiple organs which are cooled in-situ prior to their removal. This method is based on two principles. 1. Significant visceral ischemia may be produced inadvertently by dissection as a result of vasospasm, compression, or technical mishep. This is avoided through use of non-touch technique. 2. By cooling the organs in-situ via their vasculature, they are at once flushed of blood and rapidly cooled to preservation temperature. This process yields far more rapid and evently distributed cooling than surface cooling alone. The intraoperative donor management is greatly simplified because it limits operative time, blood loss and fulid shifts. With this method, the requred dissection is performed once the organs are cold and asanguinous, making small vessels difficult to identify. A careful dissection of the vascular structures to an extend that allows anatomical clarification often leads to a compromise between both principles.
