Abstrakt | The role of humoral immunity in rejection is still not clearly delineated. This study analysed 101 renal transplant biopsies from 69 patients (43 men, 26 women, age range 9-71 years) and 31 control normal kidneys by light and immunofluorescence microscopy techniques. Glomerular immune deposits were found in 88% of transplant biopsies, with suggested immune allografts. Extraglomerular vascular immune deposits were observed in 97% of renal transplant biopsies and 55% of normal kidney. igM and C3 with fine granular pattern were associated with acute vascular rejection, while homogeneous lumpy deposits occured mostly in chronic transplant nephropathy as well as non-rejection vascular lesions. Granular and rarely linear deposition of IgG, IgM and/or C3 along the tubular membrane and in interstitial capillaries were observed in renal transplant biopsies not associated with any histomorphologic group. In conclusion, immune reactant deposition is a frequent finding in kidney transplant biopsies. The observation of characteristic vascular immune deposits in acute vascular rejection is compatible with humoral mechanism of rejection. | |
