Abstract     Incidence of incisional hernia following abdominal surgical procedures is relatively high. It varies from one institution to another from 2% to 11%. Multiple factors influence the development of incisional hernia. Most frequent cause is infection of oeprative wound with clinical or subclinical inflammation. Variability and complexity of ventral incisional hernias as well as high reccurrency rate dictated introduction of numerous different surgical aproaches. Some are mentioned in this article. In six year period 205 hernia repairs of ventral incisional hernias were performed at our department. More than half occurred in vertical midline laparotomy wound. Next in frequency were hernias in subcostal wound used for cholecystectomy. Hernia repair with direct suture was most commonly performed, less frequently prosthetic repair with GoreTex patch or polypropylene mesh. There were no major postoperative complications. 18 (9%) patients suffered wound inflammation, pneumonia or headache. In 46 cases hernia recurred. Tension-free procedures and new prosthetic materials used in hernia repair encourage hope of patients as well as surgeons to lower the recurrence rate.
