Abstrakt | Background. Aseptic loosening of total hip replacements is one of the most concerning problems in orthopaedics. Loosening is a complex process dependent on numerous mechanical and biological factors. Histologic response of periprosthetic tissue is dominated by a granuloma reaction including numerous histiocytes, giant cells and fibrous tissue. Within the cytoplasm of macrophages, as well extracellularly, wear debris including polyethylene (PE), polymetilmetacrylate (PMMA) cement and metal particles are found. Activated macrophages release mediators, which induce bone resorption and, consequently, the process of loosening. Conclusion. Only by a close collaboration of orthopaedic surgeons, multidisciplinary researchers and implant manufacturers thedevelopment of implants which would enable a decrease in the incidence of loosening can be obtained. | |
Izvleček | Izhodišča. Aseptično omajanje kolčne proteze je pereč problem. Omajanje je kompleksen proces, ki je pogojen s Številnimi mehaniskimi in biološkimi dejavniki. V histološki sliki tkiva ob protezah prevladuje granulomska reakcja s številnimi histiociti in velikankami ter vezivnim tkivom. V citoplazmi makrofagov, pa tudi zunaj njih lahko najdemo obrabne delce bodisi iz polietilena (PE), delce polimetilmetakrilata (PMMA) ali pa kovinske delce. Iz aktiviranih makrofagov se sproščajo mediatorji, ki spodbujajo predvsem razgradnjo kosti, kar povzroča omajanje proteze. Zaključek. Potrebno je tesno sodelovanje raziskovalcev različnih področij in proizvajalcev endoproteze, da bi razvili take vsadke, ki bi še zmanjšali pogostnost pojava omajanja. | |
