Abstract		Indications for an urgent percutaneous transluminal coronary angioplasty (PTCA) are extending beyond mechanical consequences of the acute myocardial infarcton also to electrical consequences. Even very small areas of ischemia can cause a life threatening arrhytmia. Sometimes, as shown in our two cases, the presenting syptom of acute ischemia can be a rhythm disturbance, rather than a typical chest pain. Immediate treatment of ischemia has proven to be the best therapy for such rhythm disturbance. The limiting factor is unavailability of the method. Currently there is only one catheterization laboratory qualified to perform urgent angioplasty in Slovenia.
