Abstract     Rapid restoration of pulmonary blood flow is important in preventing death due to a massive pulmonary embolus. Usulay we use thrombolytic agents to accelerate the lysis of acute emboli. Unfortunately in certain number of patients such treatment is contraindicated or unsuccessful. We present an alternative mechanical treatment with percutaneous catheter embolectomy in two patients with massive pulmonary embolism. The first patient had contraindication for thrombolytic treatment and in the second cause the thrombolytic treatment was unsuccessful. Technique of catheter procedure and indications for alternative mechanical treatment are discussed.
