Abstrakt     Background. To study different clinical parameters to predict the success of in vitro fertilization-embryo transfer (IVF-ET) in the natural cycle. Methods: Prospectively collected data on follicular growth monitoring, laboratory data, and clinical results in 122 women with 196 attempts of IVF-ET in unstimulated cycles for tubal infertility or endometriosis were retrospectively evaluated. Results: Oocytes were retrieved in 170 (86%) of 196 cycles. There were 116 embryo transfers: 59% of all natural cycles and 68% of 170 natural cycles with oocytes retrieved. The first 28 embryo transfers were performed on day 2, and the following 88 embyo tranfers were performed on day 4 after oocyte retrieval. There were 32 clinical pregnancies; 28 healthy babies were bom at term:14% per puncture,16% per positive puncture, 24% per transfer. Clinical variables and their possible influence on the results of IVF in the natural cycle were evaluated. There were no statistical differernces in pregnancy rates per embryo transfer between higher (0.60-1.17 nmol/L) and lower (0.40-0.59 nmol/L) estradiol concentrations on the day of human chorionic gonadotropin (hCG) (30% vs. 30%), between early (10-14 d) and late (16-21 d) aspiration day(25% vs. 32%), between smaller (16-18 mm) and larger (19-22 mm) folicular diameters on the day of hCG (22% vs. 31 %), or between 28 embryo transfers on day 2 and 88 embryo transfers on day 4 after oocyte retrieval (18% vs. 32%), but there was a significant difference (p < .05) between cycles with thin (6-8 mm) versus thick (9-12 mm) endometrium on the day of hCG (20% vs. 36%). Conclusions: Low estradiol levels on the day of hCG compared to those reported for IVF-ET in stimulated cycles, and good results despite previous unsuccessful attempts at IVF-ET in stimulated cycles encourage broader acceptance of IVF-ET in the natural cycle.
