Abstrakt     Screening for coeliac disease with serum IgA, IgG antigliadin and antiendomysium antibody was performed in 315 diabetics who present a group of high-risk patients. In 11 patients coeliac disease was proven by typical jejunal mucosal changes, given a prevalence of 3.4%. There was no significant difference between groups of diabetic patients with and without coeliac disease with regard to gender, age and duration of insulin-dependent diabetes mellitus. The mean age of onset of diabetes in patients with coeliac disease (6.8+-4.6 years) was lower than in diabetic patients without coeliac disease (9.14 +- 4.14 years) , but the difference was not statistically significant. Our results showed that the detection of IgA antigliadin antibody by enzyme immunometric assay (EIA) is a reliable screening test for coeliac disease because of its high sensitivity (100%) and specificity (98%). The specificity of antiendomysium antibody determination by indirect immunofluorescence proved to be very high (98.1%), while the sensitivity was age dependent, reaching 100% in children older than two years.
Izvleček     Pri 315 otrocih s sladkorno boleznijo smo določili nivo serumskih IgA, IgG antigliadinskih in antiendomizijskih protiteles. Pri 11 bolnikih smo dokazali celiakijo z značilnimi spremembami sluznice jejunuma. Prevalenca celiakije je bila pri naših bolnikih 3.4%. Ni pa bilo pomembnih statističnih razlik med skupinama diabetikov z in brez celiakije glede na spol, starost in trajanje sladkorne bolezni. Srednja starost pojava diabetesa s celiakijo (6.8+-4.6 let) je nižja kot pri bolnikih s sladkorno boleznijo brez celiakije (9.14+-4.14 let), vendar pa razlika ni statistično pomembna. Naši rezultati kažejo, da so IgA antigliadinska protitelesa določena z encimsko imunometrično metodo (EIA) zanesljiv presajalni test za celiakijo, zaradi visoke senzitivnosti (100%) in specifičnosti 98%). Specifičnost antiendomizijskih protiteles določenih z indirektno imunofluorescenco je tudi zelo visoka (98.1%), vendar pa je senzitivnost odvisna od starosti in doseže 100% pri otrocih starejših od 2 let.
