Abstract     Of the relatively frequent lung diseases in the elderly, those manifesting with obstruction in the airways contribute most importantly to increased mortality and rising health care expenses. Physicians attempting to diagnose these disease should bear in mind that already the normal aging lung has a tendency toward increased resistance in the airways, that symptomatology and clinical presentation in the elderly could be atypical and that comorbidity, notably from cardiovascular diseases, is common. Therapeutic interventions should consider changed pharmacokinetics and increased side effects. The therapeutic effects are better controlled by quality of life tests and the 6 minutes walking test compared to standard tests of lung function. Pulmonary rehabilitation as a continuous process must prevent muscular weakness, undernutrition and depression to be successful in the management of elderly patients with obstructive lung disease.
